The goal of the Behavioral Care Advocate role is to help members and their families live their lives to the fullest by promoting recovery and resiliency via coordination and collaboration with multiple internal and external partners including members and their families / caregivers, medical, behavioral health and clinical network teams. Behavioral Health Advocates work with complex and high – risk needs members, with a goal of engaging the member and their family as applicable to assist in members realization of their goals in the treatment process, decreasing their reliance on higher levels of care, helping them to access appropriate community services, and assisting them in improving community tenure.

Primary Responsibilities:

  • Utilize advanced clinical skills to engage and motivate Members and their families via a recovery, health and wellness, family – holistic and member centric oriented approach
  • Assist members and their families / caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and services
  • Identify and remove barriers to procurement, delivery, participation in and success of services
  • Provide supportive follow – up, monitoring and education as indicated telephonically
  • Partner within the context of complex care internal teams and community and external providers to address the needs, gaps in care, and recovery goals of complex and high risk members and their families, in the context of care coordination within a holistic interdisciplinary approach
  • Provide current care alerts, information on medication adherence, updates on referral results and other priority issues to community providers, as indicated
  • Identify and address the needs of those who are high utilizers of services through case conferences and informal communications
  • Participate in telephonic meetings, as needed, with providers to review clinical performance, identify areas of clinical opportunity, and oversee course for improvements
  • Participate in the ongoing identification of members who may qualify for ancillary services
  • Coordinate the development of comprehensive support system for members with serious emotional disturbance to promote the goal of children remaining in their own homes and communities
  • Serve as an Optum behavioral health liaison and subject matter expert for Texas UnitedHealth Care Medical Partners; establish and foster positive relationships with medical team, participate in medical – behavioral integration activities and discussions
  • Accept referrals via designated processes, collaborate in evaluating available services, and coordinate necessary psychiatric and community referrals, as needed
  • Contribute to treatment plan discussions; routinely attend clinical rounds telephonically and other meetings with medical partner or external entities, when applicable
  • Partner with internal Optum Behavioral Health Services Teams to coordinate a seamless transition for members from an inpatient setting to community based services
  • Meet all contractual requirements; collaborate early and frequently with other teams in the management of cases stratified as having intensive, complex, and high-risk needs
  • Comply with all policies, procedures and documentation standards in appropriate systems, tracking mechanisms and databases
  • Identify new programs, services and gaps in market; share information with network teams
  • Maintain knowledge of provider availability and barriers to access
  • Collaborate with providers to determine acuity of mental health concerns, barriers and progress; coordinate appropriate services.
  • Other activities as assigned, including but not limited to:
    • Participate in committees, educational opportunities and training
    • Maintain workload and support business needs to meet metrics

Required Qualifications:

  • Active, unrestricted, Independent Licensed Master's degree – level clinician in Psychology, Counseling or Marriage or Family Counseling OR Licensed Ph.D., OR an RN with experience in behavioral health
  • Licenses must be active and unrestricted
  • 2+ years’ experience in a related mental health environment
  • Demonstrated ability to use a PC in a Windows environment, including email and Microsoft Office applications
  • Ability to type and talk simultaneously
  • If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders

Preferred Qualifications:

  • Medical / Behavioral setting experience (i.e. hospital, managed care organization, or joint medical/behavioral outpatient practice)
  • Bilingual (Fluent in English and Spanish)
  • Pediatrics experience
  • Experience working with members across the age continuum
  • Dual diagnosis experience with mental health and substance abuse
  • Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
  • Experience working with low – income populations
  • Managed care and utilization management experience a plus
  • Excellent customer service, interpersonal and problem – solving skills
  • Solid team player and team building skills
  • Able to function independently and responsibly with minimal supervision
  • Maintain direct and open communication with all levels of the organization
  • Able to handle sensitive issues with peer, members and providers in a confidential manner
  • Demonstrate initiative in achieving individual, team, and organizational goals and objectives
  • Ability and flexibility to assume responsibilities and tasks in a constantly changing work environment
  • Solid oral and written communication skills — specifically telephone skills being able to talk and type at the same time
