Job Description –PERDIEM
PURPOSE OF YOUR POSITION:
Provides and assists in the delivery of rehabilitation programs as outlined by, and under the supervision and direction of the Rehabilitation Director.
Physical Therapist/Occupational Therapist:
Evaluates residents and develops appropriate care plans.
Provides direct resident treatment in accordance with an established care plan.
Participates in clinical program development and implementation.
Consults with and trains residents and caregivers.
SCOPE OF RESPONSIBILITY
Directly responsible to the Program Manager for each patient/resident assigned to the Rehabilitation Technician, Physical Therapist and/or Occupational Therapist. The intensity of supervision is dependent on the nature of the skilled tasks required and the complexities of the patients/residents presenting condition.

Must be able to read, write, speak and understand the English language.
Rehabilitation Technician must be a registered certified nurses’ aide in accordance with laws of this state. CPR certification preferred, but not necessary. High School graduate or equivalent preferred.
Physical Therapist or Occupational Therapist must have a State of Connecticut license in good standing.
