Peer Advocate Responsibilities
- Identify individual client needs and documents intervention based on relationship and assessment
- Provide support to clients through a variety of settings and assists with support groups, as needed
- Contribute to case management meetings to ensure team approach is delivered to each client
- Complete documentation for client contacts, touches, cancellations and care plan
- Communicate professionally with clients, families, physicians, community agencies and office staff
- Responsible for achieving set goals/Key Performance Indicators (KPIs)
- Represent and promote 180 through participation and interaction with the community
- Participate in fair, timely and respectful conflict resolution
- Support all members of the 180 team in the performance of their duties
- Participate in problem-solving and performance improvement processes
- Travel could exceed 50% of the time and include occasional overnights
Peer Advocate Qualifications
- Must be eligible to obtain Certification as a Peer Recovery Specialist
- Minimum high school diploma or equivalent
- Reliable transportation and valid driver’s license
- Basic computer proficiency; must be able to type and navigate Cloud-based environment
- 1+ year(s) as a Certified Peer Recovery Specialist preferred
- A background in working in a maternal population a plus
