13. GENERAL SUMMARY OF DUTIES/RESPONSIBILITIES

Facilitates efficient and respectful coordination of donated dental and medical services for APA patients by effectively communicating with referring dental, specialty, and primary care providers who donate services through APA. Serves as the primary contact for donating providers by managing pledges and working directly with providers’ administrative staff to ensure continuity of care for APA patients.
Responsible for reviewing dental and medical referral information to ensure accuracy of documentation prior to enrolling patients and scheduling donated care. Maintains a structured rotation of providers to ensure provider pledges are matched with APA patients needing care and to ensure the provider rotation corresponds with pledges for service. Assists with APA patient enrollments as well as trains staff in PCC tasks as needed. Maintains provider and patient databases. Provides the community with appropriate education and materials related to oral health.

Objectives of the position:

  • Facilitate an efficient process for dental, medical, and ancillary care providers to donate their services through APA to patients meeting the eligibility requirements;
  • Ensure donating providers’ commitments are adhered to when coordinating specialty and/or dental care;
  • Maintain and enhance communication with primary care clinics referring patients to APA through regular contact with administrative staff to ensure efficient referral processes.;
  • Facilitate a positive, well organized experience for APA patients to receive donated care utilizing all necessary resources such as interpreter services, transportation assistance, medication assistance, lab services, durable medical equipment, and links to additional services such as housing, food assistance, employment, etc.;
  • Ensure best practices are applied to all program procedures from reviewing referral information, to adhering to HIPAA regulations to coordinating donated specialty and/or dental health care to maintaining timely and accurate electronic health records; and
  • Contribute to continuous process improvements by taking an active role in all APA service enhancements and supporting other APA roles as needed.

14. ESSENTIAL DUTIES OF THE JOB

35% Weekly Accurately enter and manage all donating medical and dental provider information into the CARES Provider database as received and add or remove providers to maintain an accurate and current list of providers

Daily Adhere to donating provider’s health care pledges by ensuring the appropriate application and rotation of providers’ services

Daily Thoroughly review and process incoming faxes from referring providers and follow the Referral Guidelines for assessing the specialty and/or dental care being requested to determine appropriate course of action

Daily Prioritize and coordinate patient appointments for patients enrolled in
APA based on level of urgency

Daily Schedule specialty, dental, and ancillary care appointments following policies and procedures, checklists, and required protocols to ensure excellent care

15% Weekly Utilize all resources to assist patients such as interpreter services, financial assistance, transportation, referrals to housing, employment, food assistance, etc., to ensure additional barriers to health care are addressed

Weekly Assist participants to apply for ACA and/or Medicaid health insurance

Weekly Collaborate with all relevant parties including APA staff, referring providers’ care coordination team, primary care clinics, schedulers, interpreters, etc., to ensure comprehensive, whole person care is provided to APA patients

Weekly Initiate and maintain frequent communication with donating providers’ administrative staff to coordinate patient care and inquire about APA procedures and customer service satisfaction for continuous quality improvement

Quarterly Foster and maintain positive relationships with donating providers, referring clinic providers and staff to facilitate positive outcomes for patients

10% Bi-monthly Develop and present educational materials, PSAs, presentations, etc. to promote oral health and APA in the community

Ongoing Identify and communicate gaps in specialty and dental care to facilitate provider recruitment efforts and/or recommend system improvements

Quarterly/as Assist other CHA dbas with dental health coordination needed

15% Daily Respond to all phone messages within 1 business day when possible

Daily Review and record all patient data in CARES database in a timely, accurate and complete manner

  • Must have certification or associate degree in office administration, medical administration or medical assisting, dental services, human services, or related fields
  • Must have a minimum of 1-year dental or medical experience in a health-related work setting
  • May substitute three years of medical/dental or social service experience for educational requirements
  • Certified Medical Assistant or demonstrated knowledge of medical terminology
  • Documented experience reviewing electronic medical and/or dental records and chart notes
