Please note: The person who occupies this position will be employed by Gundersen Tweeten Care Center, located in Spring Grove, MN.
Acts as a resource to residents, staff and families regarding community, legal, financial, and supportive care services. Ensures Social Work records and documentation are in compliance with State and Federal rules and regulations. Maintains an effective system to gather resident information before, during and after admission to ensure the individual needs of each resident are valued while working in a manner consistent with all current federal, state, and long term care regulations.
Ability to work in close collaboration with physicians and other health care personnel in resident evaluation and treatment to further their understanding of significant social and emotional factors underlying resident’s health problem.
Ability to ensure PASAAR is completed prior to admissions for all residents.
Ability to counsel residents and family in dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.
Ability to document the social care component of the Comprehensive Care Plan
Ability to identify potential residents whose needs can be met by the facility by receiving and returning phone calls from referral agents in an efficient manner; gather sufficient pre-admission data including history and physical, medication lists, therapy and nursing notes and insurance information, to determine resident level of care and the ability of the facility to properly care for the resident
Ability to explain resident’s rights to residents and resident’s representatives.
Ability to educate residents and families on Advanced Directives, guardianships and Durable Power of Attorney and Do Not Resuscitate forms.
FT role, weekdays rotate and every other weekend (evenings or overnight), typically 8 hour shifts, part time roles available as well.
Bachelor’s Degree in Social Work or Human Services field; or one to two years related experience and/or training; or equivalent combination of education and experience.
