.Full Time- behavior health intake/assessment specialist needed immediately to work in hospital emergency department setting. Candidate will work closely with a multi- disciplinary team under the direction of the Program Supervisor and hospital staff. The intake/assessment specialist will work closely with the ED medical staff in linking patient seen in the ED with available community health services to include SUD treatment, mental health, primary medical care and housing services. Specific duties include but are not limited to: screening, triage, assessment and linked referrals to available community resources. Person should be familiar with basic behavioral screening tools (AUDIT, CAGE, etc.), community referrals and how to conduct case management activities. Individuals experience with working with patients impacted by SUD, MH and/or homelessness strongly encouraged to apply. Strong team player, multi-tasker and excellent communication skills a must.
