Qualifications & Description: Provide ongoing counseling, case management, and coordination of support services to adults with developmental disabilities in a vocational training program by making and documenting regular and frequent observations. Act as an Interdisciplinary Team chair to facilitate annual ISP reviews, informal reviews and ongoing evaluations. The Counselor acts as a primary liaison between DHE, the consumer (program participant) and the consumer’s family, care-provider, Regional Center representative, licensing representative, trainers/instructors, etc. The Counselor conducts both intake and initial evaluations, ongoing evaluations, and discharge meetings for consumers. Minimum one year experience working with individuals with intellectual and developmental disabilities or similar population and responsibilities. Certificate or degree in Sociology, Psychology or similar studies/disciplines required. Bilingual in Spanish strongly preferred. Familiarity with counseling methodologies. Ability to exercise sound judgment and to maintain a high level of confidentiality. Must be able to establish and maintain cooperative internal and external working relationships. Excellent verbal and written communication skills required. Must have excellent organizational skills and the ability to establish and maintain necessary files, documents, case notations, and reports. Must have valid CA driver license and clean driving record. Must pass fingerprint and drug test. CPR/First Aid Certification required. Must be bilingual (Eng./Spn.)

General Duties: This is a broad/general overview of the job duties of the position, which include, but are not limited to, providing ongoing counseling, case management, and support services for consumers within internal programs. Address consumer-related matters; conduct admission meetings, annual review meetings, corrective action meetings, exit interviews with terminating/exiting consumers and prepare discharge summaries. Conduct and facilitate meetings as described above. Additional duties as needed and directed.
