Grace Hospital is a not-for-profit registered Acute Long-Term Care Hospital, the first of its kind to be established in Cuyahoga County. We are Medicare and Medicaid certified and fully accredited by the Joint Commission. Grace Hospital utilizes an interdisciplinary care team model to produce optimal clinical outcomes. We seek clinicians who are compassionate, competent, professional and results-oriented to join our team.

Job Summary: An exciting opportunity for a Monday-Friday Daytime position in a professional setting assisting patients transition from short-term to post-acute care, including inpatient rehabilitation, skilled nursing, and home care. A Clinical Liaison performs pre-admission evaluations including reviewing charts and interviewing patients, family members, medical staff, and case managers to determine appropriateness for transfer and admission. The Clinical Liaison assists in identifying any special needs and in coordinating transfer of patients. As needed, financial information will be collected and insurers contacted.

Qualifications:

Experience: Licensed as a Registered Nurse or Respiratory Therapist in the State of Ohio. Case management or utilization management experience is desirable. Must be self-motivated with high professional standards; have the ability to effectively interact as a team member; and demonstrate excellent written and verbal communication skills. A valid driver’s license and proof of insurance are required. Previous experience as a Registered Nurse or Respiratory Therapist.

Responsibilities:

  • Performs pre-admission evaluations including reviewing charts and interviewing patients, family members, medical staff, and case managers.
  • Assists determine appropriateness for transfer and admission.
  • As needed, collects financial information and communicates with insurers.
  • Assists in identifying any special care needs and in coordinating transfer of patients.
  • Develops and maintains productive working relationships with physicians, case managers, discharge planners, and social workers.
  • Educates physicians, patients, families, and referral sources about Long Term Acute Care services.
  • Conducts tours of facilities and services when needed.
  • Achieves positive feedback from physicians, patients, family members, and referral sources.
  • Identifies and recommends procedures and policies for enhancing clinical and support services
