Job Summary
- To provide quality, compassionate services in accordance with the client’s program and care plans. The primary goals are to promote consumer dignity, independence, comfort, mobility, personal appearance, and safety.
Duties/Responsibilities
- At the direction of the office Assistant Agency Manager and/or the office Schedulers the Care Specialist will receive their various assignments daily. Schedules can and will be adjusted on a last-minute basis depending on the needs and clients
- Based on program of the client being assigned the following duties will be performed:
HCBS/Private Pay/VA Clients
- Performs homemaking and personal care duties in the client’s home, during the hours and on the days assigned
- Assists the client with other needs as outlined by the care plan
- Recognizes the importance of a clean, healthy environment in performing household tasks and takes measures to keep the home in safe condition
- Protects and respects the confidentiality of all client records and relationships and keeps all client personal health information according to policy
- Completes requisite paperwork and documentation on time and accurately
- Attends training applicable to the duties performed and makes other training needs known to the office staff
- Follows agency policies and procedures
- Communicates with the appropriate office personnel on client needs and assists in coordinating care with family members
- Performs other duties as assigned
IHSS Clients
- In the consumer’s home performs skilled attendant, personal care, and homemaking duties outlined in the care plan, during the hours and on the days assigned by the consumer or authorized representative
- Recognizes the importance of a clean, healthy environment in performing household tasks and takes measures to keep the home in a safe condition
- Protects and respects the confidentiality of all client records and relationships and keeps all client personal health information according to policy
- Completes required paperwork on time and accurately
- Attends training applicable to the duties performed and makes other training needs known to the office staff
- Follows state regulations and office policies and procedures
- Communicates with the appropriate office personnel on consumer needs
- Immediately reports all incidents to the office
- Performs other duties as assigned by the consumer that fall within state regulations, the policies of this agency and as time allows
- Caregiver Check Offs – After completion of training Care Specialist will, under the supervision of the RN’s, conduct caregiver check off’s in the homes when needed.
- Setting up equipment for Video Observation of client visits.
- Coverage Assurance. In conjunction with schedulers respond to situations where scheduling conflicts, emergencies and/or call outs arise that could result in a missed visit. Ensure that Private Pay clients are started immediately with care. Work with clients who are not utilizing full hours due to lack of caregiver.
- Miscellaneous: Perform other functions as directed by the Supervisor, Agency Manager, and/or the Home Care Administrator (Items may include, but not limited to filing, phone coverage or special projects).

Education
- High school/GED or equivalent desirable
- Basic First Aid and CPR preferred, but not required
Experience
- Experience in the field of home health care is preferred but not required
- Experience managing a home and family or caring for disabled individuals is extremely desirable
- Current driver’s license required

- Prolonged periods of sitting at a desk and working on a computer.
- Essential physical requirements, such as standing, kneeling, squatting, stooping, or typing.
