Bilingual Case Manager – Brooklyn, NY 11216

  • Assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress
  • Admits new clients by reviewing records and applications
  • Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation
  • Monitors cases by verifying clients’ attendance, observing and evaluating treatment and responses; intervening in crises
  • Maintains clients’ records by reviewing case notes; logging events and progress
  • Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations
  • Handles intakes of adult couples and ensures that all documentation is accurate and properly filed
  • Recommends action plan and creates ILP for clients
  • Develops roster of social and other services for client agreement
  • Responsible for weekly room inspections
  • Maintains detailed records of each client in the form of progress notes through CARES
  • Provides Director of Social Services with reports and summaries of caseload for management review
  • Responsible for meeting target goals set for each Case Worker/Housing Specialist
  • Keeps Director of Social Services and management informed of client progress
  • Liaison for management, communicating clients’ progress and any case concerns

· SKILLS AND QUALIFICATIONS

  • Bachelor’s Degree required psychology, social work, sociology, mental health, nursing, health and human services, or education services.
  • Minimum of 2 years’ experience working within the Social Services Field
  • Experience with DHS Cares / Homeless population is preferred
  • Bilingual English/Spanish is preferred
